Eligibility to test requires verification of CNA training using one of the following four (4) choices:
- If you have successfully completed a Tennessee Department of Health approved
Training Program within the last two years, submit a copy of the
Registration Roster from your training program to D&S DT, and on
the registration page enter the ID for your program from the following list.
- If you are enrolled in a Nursing Program and have satisfactorily completed a
Fundamentals of Basic Nursing course with a clinical component or have
a Military related nursing background, then submit your Validated Challenge
approval letter issued by the Tennessee Department of Health to D&S DT, and on
the registration page enter [9997] as your training program ID.
- If you are taking the examination because your CNA certification on the
Tennessee registry lapsed (a previously certified Tennessee Nursing Assistant),
then submit a copy of the Registration Roster from the training program that you
completed within the last two years to D&S DT, and on
the registration page enter the ID for your program from the following list.
- If you are currently certified in a State for which Tennessee does not
accept reciprocity, then submit your Validated Challenge Approval
letter issued by the Tennessee Department of Health to D&S DT, and on
the registration page enter [9997] as your training program ID.

Note: Your test results will not be released and you will not be placed on
the Nurse Aide Registry until your Registration Roster or Validated Challenge
approval letter is received at D&S DT.
