Attention: Once your credit is approved, you may apply and test. However, proof of training must be mailed or faxed directly to Headmaster/D&S Diversified Technologies, and you will not receive test results and you will not be placed on the registry until that proof of training is received.

You will first be asked to complete our on-line application in its entirety. Note that If you answer Yes to selected Background Questions you will be required to submit documentation directly to the Arkansas State Board of Nursing. Please note and record the pin number on your initial page. It will be required for future access to your record. Upon successful completion of the application, you will be asked for your credit card information. Your exam costs will be automatically calculated based on your request. If your credit is rejected you may go BACK and re-enter the credit information. Note that your record will not be saved unless your credit is successfully verified. Once your credit is accepted you may select a test event. Note that should you pass the exam, your record will not be forwarded to the state registry until we have recevieved proof of training.
